| Derived fact | A deterministic transformation of source facts. |
| Model inference | An output produced through a model process. |
| Recommendation | A proposed action, not an executed action. |
| Unknown | A required fact that is not established by available evidence. |
| Coverage | A canonical representation of insurance coverage context. |
| Authorization | An approval/authorization record or context when represented by source data. |
| Payer | The payer organization associated with claim/coverage activity. |
| Canonical record | Vendor-neutral DenialOS representation of source data. |
| Source record | Original upstream representation. |
| Provenance | Metadata describing where a DenialOS fact came from. |
